Patient Safety Tools: Improving Safety at the Point of Care--Toolkit and Resource Descriptions
AHRQ grantees have produced 17 patient safety toolkits. A number of these toolkits focus on improving communication with patients, such as the following toolkit that was tested with diverse patients with low health literacy.
Project RED (Re-Engineered Discharge). Use this toolkit to redesign the discharge workflow using a set of 10 discrete, mutually reinforcing components that aim to reduce post-discharge adverse events and subsequent re-hospitalizations. Two features of the re-engineered process are a discharge advocate who works with patients throughout the process, and the real time production of a simple, easy-to-understand discharge plan. Find more information about Project RED and the toolkit at: http://www.ahrq.gov/qual/pips/grants.htm#jack.

On Tuesday March 31, 2009 from 2:00 pm - 3:15 pm (EDT) AHRQ will host a free Web conference on Project RED. Learn more about the intervention that led to a 30 percent reduction in hospital readmissions and emergency room visits.
