Medication discrepancies at pediatric hospital discharge.



    Medication reconciliation at discharge remains a patient safety challenge. In this prospective study of pediatric inpatients, a significant proportion of records reviewed had at least one medication discrepancy between the discharge record and subsequent parent report and pharmacy records. The implementation of electronic health records has not solved the issue of medication discrepancies at discharge. It is likely that a systems approach will be needed to address this problem.
