MN Community Measurement

• Depression care: percentage of patients 18 years of age or older with major depression or dysthymia who demonstrated a response to treatment 12 months (+/- 30 days) after an index visit.

• Depression care: percentage of patients 18 years of age or older with major depression or dysthymia who demonstrated a response to treatment six months (+/- 30 days) after an index visit.

• Depression care: percentage of patients 18 years of age or older with major depression or dysthymia who reached remission 12 months (+/- 30 days) after an index visit.

• Depression care: percentage of patients 18 years of age or older with major depression or dysthymia who reached remission 6 months (+/- 30 days) after an index visit. This updates a previously published measure summary.

• Diabetes mellitus care: percentage of patients 18 to 75 years of age who had a diagnosis of type 1 or type 2 diabetes and whose diabetes was optimally managed during the measurement period. This updates a previously published measure summary.

• Vascular care: percentage of patients 18 to 75 years of age who have IVD with optimally managed modifiable risk factors. This updates a previously published measure summary.
