Data Submission for AHRQ SOPS™ Community Pharmacy Survey 

Community pharmacies that have administered AHRQ’s Community Pharmacy Survey on Patient Safety Culture can submit their data to the Database October 1 – October 23. Survey results will be available in the SOPS Community Pharmacy Database Report. The report will help community pharmacies assess their own results to identify strengths and opportunities for improvement by providing average scores and percentiles on the survey items and composite measures. Participation is voluntary.

Community pharmacies who have not previously administered the survey may still do so. The survey and supporting materials are available at https://www.ahrq.gov/sops/quality-patient-safety/patientsafetyculture/pharmacy/index.html.
