Introduction of a mobile adverse event reporting system is associated with participation in adverse event reporting.

Physicians historically are low utilizers of voluntary error reporting systems. In this study, the investigators developed a smartphone-based application for voluntary reporting of anesthesia errors. Availability of the application increased physician reporting of errors over a 3-year period.
