The Pursuing Excellence Collaborative: engaging first-year residents and fellows in patient safety event investigations.
https://psnet.ahrq.gov/issue/pursuing-excellence-collaborative-engaging-first-year-residents-and-fellows-patient-safety
Previous research suggests that residents may underutilize adverse event reporting tools. This article describes an 18-month clinical learning collaborative among 16 sites intended to increase resident and fellow participation in patient safety event investigations. Researchers found the collaborative increased participation in event investigation and improved the quality of the investigation.
Mind the power gap: how hierarchical leadership in healthcare is a risk to patient safety.
https://psnet.ahrq.gov/issue/mind-power-gap-how-hierarchical-leadership-healthcare-risk-patient-safety
Hierarchy as an organizational or team structure is known to affect patient safety. This editorial examines the impact a strict chain of command can have on communication and awareness-raising actions in the care environment. The author illustrates how a staff’s ability to contribute to care goals and raise concerns is enhanced when all are respected as having valuable insights without deference to role or education level.
Quality and safety practices among academic obstetrics and gynecology departments.
https://psnet.ahrq.gov/issue/quality-and-safety-practices-among-academic-obstetrics-and-gynecology-departments
Staff engagement in safety and quality improvement efforts fosters a culture of safety and can reduce medical errors. This survey of 52 obstetrics and gynecology departments at academic medical centers found that few departments provided faculty with protected time or financial support for quality improvement activities, and only 5% of departments included a patient representative on the quality committee.
Opportunities to improve diagnosis in emergency transfers to the pediatric intensive care unit.
https://psnet.ahrq.gov/issue/opportunities-improve-diagnosis-emergency-transfers-pediatric-intensive-care-unit
Improving diagnosis in pediatrics is an ongoing patient safety focus. This retrospective study included 129 pediatric emergency transfer cases and examined the relationship between missed opportunity for improvement in diagnosis (MOID; determined using SaferDx) and patient outcomes. Researchers found that MOID occurred in 29% of emergency transfer cases and it was associated with higher risk of mortality and longer post-transfer length of stay.v
Healthcare fragmentation, multimorbidity, potentially inappropriate medication, and mortality: a Danish nationwide cohort study.
https://psnet.ahrq.gov/issue/healthcare-fragmentation-multimorbidity-potentially-inappropriate-medication-and-mortality
System weaknesses (e.g., resource availability, deficiencies in care coordination) threaten patient safety. This population-based cohort study including 4.7 million Danish adults who interacted with primary or hospital care in 2018, found that indicators of care fragmentation (e.g., higher numbers of involved clinicians, more transitions between providers) increased with patient morbidity level. The researchers found that higher levels of care fragmentation were associated with adverse outcomes, including potentially inappropriate prescribing and mortality.
Exploring the impact of safety culture on incident reporting: lessons learned from machine learning analysis of NHS England staff survey and incident data.
https://psnet.ahrq.gov/issue/exploring-impact-safety-culture-incident-reporting-lessons-learned-machine-learning-analysis
Voluntary incident reporting rates may be an indicator of organizational safety culture. Using different machine learning algorithms, this study found that several components of safety culture – compassionate culture, violence and harassment, and work pressure – have a significant impact on predicting incident reporting behavior.
Exploring medication safety structures and processes in nursing homes: a cross-sectional study.
https://psnet.ahrq.gov/issue/exploring-medication-safety-structures-and-processes-nursing-homes-cross-sectional-study
Effective implementation of health information technology can promote medication safety. This survey of 118 nursing homes in Switzerland found that organizations employ a variety of electronic health record (EHR)-based tools to support medication safety, such as standardized medication lists, alerts for potentially inappropriate prescribing, or electronic data exchanges with community pharmacies or outside physicians.
Estimating breast cancer overdiagnosis after screening mammography among older women in the United States.
https://psnet.ahrq.gov/issue/estimating-breast-cancer-overdiagnosis-after-screening-mammography-among-older-women-united
Overdiagnosis can result in overtreatment, physical harm, and emotional distress. Using SEER-Medicare data, researchers examined breast cancer overdiagnosis by comparing cancer incidence among women who discontinued mammography screening after age 70 compared to women who continued to receive screening mammograms. Findings suggest that breast cancer may be potentially overdiagnosed among 31% of women aged 70 to 74 years, 47% of women aged 75 to 84 years, and 54% of women aged 85 and older who continue to receive screening mammograms.
Effect of lawsuits on professional well-being and medical error rates among orthopaedic surgeons.
https://psnet.ahrq.gov/issue/effect-lawsuits-professional-well-being-and-medical-error-rates-among-orthopaedic-surgeons
Previous analyses have found that orthopedic surgery is one common source of patient harm. This survey of 305 orthopedists found that involvement in a medical malpractice lawsuit within the past two years increased the likelihood of experiencing burnout and reporting a medical error resulting in patient harm in the past year.
Activating pharmacists to reduce the frequency of medication-related problems (ACTMed): a stepped wedge cluster randomised trial.
https://psnet.ahrq.gov/issue/activating-pharmacists-reduce-frequency-medication-related-problems-actmed-stepped-wedge
Medication safety in ambulatory care settings is an area of growing concern. This article describes ACTMed (ACTivating primary care for MEDicine safety), a cluster randomized trial set in Australia which intends to improve medication safety in primary care settings. The ACTMed intervention will use health information technology (e.g., clinical indicator algorithms), guideline-based clinical recommendations, shared decision-making, and financial incentives to reduce serious medication-related harm, medication-related hospitalizations, and death.
A natural language processing approach to categorise contributing factors from patient safety event reports.
https://psnet.ahrq.gov/issue/natural-language-processing-approach-categorise-contributing-factors-patient-safety-event
Analyzing patient safety incident reports is essential to organizational learning, but comes with both a time and financial burden. This study found that natural language processing can be used to process unstructured patient safety event reports and reduce the burden of manually identifying and extracting factors contributing to the event.
Anesthesia Risk Alert program: a proactive safety initiative.
https://psnet.ahrq.gov/issue/anesthesia-risk-alert-program-proactive-safety-initiative
Lee B, Marhalik-Helms J, Penzi L. Jt Comm J Qual Patient Saf. 2023;49(9):441-449.
Perioperative and anesthesia care present unique patient safety challenges. This article describes the development and implementation of the Anesthesia Risk Alert (ARA) program, which promotes collaborative clinical decision-making and recommends risk mitigation strategies to address specific high-risk clinical scenarios. Since implementation began in 2019, ARA compliance has exceeded 90% and has reduced the rate of adverse events among certain high-risk patients, such as those with a high body mass index
Diagnostic reliability in teledermatology: a systematic review and a meta-analysis.
https://psnet.ahrq.gov/issue/diagnostic-reliability-teledermatology-systematic-review-and-meta-analysis
Bourkas AN, Barone N, Bourkas MEC, et al. BMJ Open. 2023;13(8):e068207.
Telemedicine can improve access to specialist care and reduce time to treatment. This systematic review including 44 articles examined the diagnostic agreement between teledermatology and face-to-face consults. The overall average diagnostic agreement was 68.9%, but subgroup analyses identified significantly higher agreement when dermatologists conducted face-to-face and teledermatology consults, rather than non-specialists (i.e., primary care or emergency medicine physicians).
Just what the doctor ordered: missed ordering of venous thromboembolism chemoprophylaxis is associated with increased VTE events in high-risk general surgery patients.
https://psnet.ahrq.gov/issue/just-what-doctor-ordered-missed-ordering-venous-thromboembolism-chemoprophylaxis-associated
Baimas-George MR, Ross SW, Yang H, et al. Ann Surg. 2023;278(3):e614-e619.
Hospital-acquired venous thromboembolism (VTE) remains a significant source of preventable patient harm. This study of 4,252 high-risk general surgery patients found that only one-third received care in compliance with VTE prophylaxis guidelines. Patients receiving guideline-compliant care experienced shorter lengths of stay (LOS), fewer blood transfusions, and decreased odds of having a VTE, emphasizing the importance of initiating VTE chemoprophylaxis in high-risk general surgery patients.

The Promise and Challenges of Implementing Pharmacogenomics to Improve Population Health: Where Are we Heading with Preemptive Pharmacogenomic Screening?
The Promise and Challenges of Implementing Pharmacogenomics to Improve Population Health: Where Are we Heading with Preemptive Pharmacogenomic Screening?
September 14, 2023, 12:00 pm – 1:00 pm ET
Individual variability in drug response efficacy or safety is becoming an increasing problem globally, given the growing population of older adults. Reasons for this variability include genomic factors, an area of study called pharmacogenomics. With genotyping technologies now widely available and decreasing in cost, implementing pharmacogenomics into clinical practice is widely viewed as an initial step in mainstreaming genomic medicine. The benefits of pharmacogenomic testing before starting drug therapy has been well documented for several single gene-drug combinations. In addition, the clinical utility of a pre-emptive genotyping strategy using a pharmacogenetic panel is being rigorously assessed. However, major challenges of implementation lie at the point of integration into healthcare systems, including the modification of clinical pathways and a large knowledge gap in pharmacogenomics in the healthcare workforce. Pharmacogenomics can also be used in a broader sense for drug discovery and development, with increasing evidence suggesting that genomically defined targets have an increased success rate during clinical development. Our speaker will review recent developments in the field of pharmacogenomics and the ongoing challenges in equitable translation of discoveries into clinical practice and public health programs.

https://psnet.ahrq.gov/issue/automated-search-methods-identifying-wrong-patient-order-entry-scoping-review
Understanding causes of wrong-patient order entry (WPOE) can help develop interventions to reduce those medication errors. This review summarizes how organizations and providers identify WPOE, what data are being captured, and causes. The most common organizational detection method is the retract-and-reorder method whereby a medication order is cancelled then reordered on a different patient within a specified period of time. There was minimal data on how providers detect their own WPOE errors. Technology and physician workload were identified as contributors to WPOE.
A multi-facetted patient safety resource--a qualitative interview study on hospital managers' perception of the nurse-led Rapid Response Team.
https://psnet.ahrq.gov/issue/multi-facetted-patient-safety-resource-qualitative-interview-study-hospital-managers
Rapid response teams (RRT) are activated, typically by nurses, when a patient demonstrates signs of imminent clinical deterioration, in order to prevent death or transfer to the intensive care unit (ICU). This study asks ICU managers about their perceptions of RRT beyond the stated goal of preventing patient deterioration. They describe the RRT as providing valuable education for new nurses and physicians and enhancing cohesion between the ICU and other wards. However, nurse managers stated they wanted more data and feedback from executive leadership.

Reducing central line-associated bloodstream infections (CLABSI) is an important patient safety improvement target. This study examined the relationship between compliance with evidence-based CLABSI guideline bundles and CLABSI rates in 159 hospitals. Between 2011 and 2021, researchers found that adherence to bundle guidelines was associated with a significant reduction in CLABSI rate.
Clinicians' insights on emergency department boarding: an explanatory mixed methods study evaluating patient care and clinician well-being.

Loke DE, Green KA, Wessling EG, et al. Jt Comm J Qual Patient Saf. 2023;Epub Jul 2.
Quantitative studies have demonstrated that emergency department (ED) overcrowding and patient boarding contribute to adverse events. This study includes both qualitative and quantitative methods to understand how ED clinicians view the impact of boarding on their own well-being as well as patient safety. Key themes include clinician dissatisfaction and burnout, and high rates of verbal and physical abuse from boarding patients. Possible solutions included improved standardization of care, proactive planning, and culture change hospital-wide.
Common contributing factors of diagnostic error: a retrospective analysis of 109 serious adverse event reports from Dutch hospitals.
https://psnet.ahrq.gov/issue/common-contributing-factors-diagnostic-error-retrospective-analysis-109-serious-adverse-event
Hooftman J, Dijkstra AC, Suurmeijer I, et al. BMJ Qual Saf. 2023;Epub Aug 9.
Diagnostic errors are common and have many contributing factors. This study analyzed more than 100 serious adverse event (SAE) reports in acute care using four investigation methods (e.g., Diagnostic Error Evaluation Research (DEER) taxonomy, Safer Dx Instrument) to identify common contributing factors. Transitions of care were particularly vulnerable to SAE, often due to incomplete communication between departments. Diagnostic errors occurred most often in the testing, assessment, and follow-up phases, with human factors as the most common contributing factor. Using multiple investigative methods supports more targeted interventions in each phase of diagnosis.

As the heat of the summer reaches its peak, and (hopefully soon) gives way to the cooler autumn breezes, it’s time for children and youth to return to school. As we prepare for the new academic year, it's crucial to remember that school readiness isn't just about having the right supplies, it's also about mental health. SAMHSA is committed to promoting mental health and preventing substance misuse as a critical part of overall health and wellness. As part of this commitment, we want to equip parents, teachers, and students with resources and tools to foster a mentally healthy return to school.
