Reducing errors through discharge medication reconciliation by pharmacy services.

In this observational study at a large academic medical center, pharmacists found medication discrepancies in 41% of patients discharged by the hospitalist service. The authors advocate for involving pharmacists in discharge medication reconciliation to avoid adverse drug events, though this strategy has previously yielded mixed results.
