Investigating Interventions to Reduce Unsafe Use of Antibiotics

Investigating Interventions to Reduce Unsafe Use of Antibiotics: Larissa Grigoryan, M.D., Ph.D.Associate Professor of Family and Community MedicineBaylor College of Medicine

Antibiotic Stewardship in the Neonatal Intensive Care Unit

https://pubmed.ncbi.nlm.nih.gov/38835250/ Antibiotics are often given to preterm and vulnerable newborns before infections are confirmed in neonatal intensive care units (NICUs). In an article published in the Journal of Intensive Care Medicine, AHRQ-supported scholars reviewed general principles of antibiotic stewardship, a careful approach to reducing unnecessary antibiotic use, and how it can be applied to NICUs to avoid adverse effects and limit antibiotic resistance. They summarized current antibiotic use metrics and reviewed evidence-based infection prevention practices in the NICU. They also provided current recommendations for practical antibiotic use in the NICU, summarized the usefulness of infection biomarkers and highlighted specific threats in the NICU. They concluded that by implementing these practices, NICUs can better protect newborns from risks associated with overusing antibiotics, improve patient outcomes and help combat the global issue of antibiotic resistance.

The page links to NCTSN resources related to young people experiencing homelessness, including fact sheets for service providers working with children and families experiencing homelessness and adaptations of Psychological First Aid for young people and families experiencing homelessness. Another resource for supporting youth experiencing homelessness, and youth in general, is the Family & Youth Services Bureau (FYSB), part of the U.S. Department of Health and Human Services’ Administration for Children and Families. FYSB supports organizations that work to promote youth well-being, prevent and end youth homelessness, and support a holistic adolescent approach by fostering collaborative partnerships across communities; leading in partnership with youth and young adults; promoting diversity, equity, inclusion, and accessibility; and supporting data-driven practices. Among FYSB’s programs is the Runaway and Homeless Youth Program. FYSB also funds and partners in providing services with the National Runaway Safeline (NRS), which offers a crisis hotline and online services 24 hours a day, 365 days a year to youth at risk of running away, those who have run away or are homeless, and their families. NRS offers services including crisis intervention, information and referrals, three-way conference calls, and more. Medicare moves to expand coverage for weight-loss drugs Wegovy, Zepbound It’s unclear how the incoming Trump administration, which would have to finalize the rule, will view it

Medicare currently covers the medications made by Novo Nordisk and Eli Lilly to treat diabetes and heart disease, but not for obesity alone because it’s prohibited by law from doing so. Given the price tag, a bill in Congress to extend coverage has stalled for years. The White House estimates the new proposal would provide coverage to an estimated 3.4 million older adults in the Medicare program, and 4 million people in Medicaid. The proposal would go into effect in 2026, but it's unclear whether the incoming Trump administration would support such a measure, or would work to roll it back.

Trump’s promise to reinstate travel bans has scientists deeply concerned Universities are cautioning foreign students to assess winter break travel plans

With his reelection, members of the life sciences community are concerned that Trump will make good on his campaign promise to reinstate and expand travel restrictions put in place during his first term. Travel limitations on international researchers could further stress the academic workforce, which is experiencing an unprecedented exodus of early-career life scientists, a trend that has already caused some projects to slow or stall. Kennedy Jr. to head the Department of Health and Human Services — the MAHA movement has taken center stage. That, however, doesn’t mean that all its adherents fit under a neat political umbrella. In fact, the MAHA (short for “Make America Healthy Again”) movement has become a home for the disaffected of various stripes, whether that’s a middle school teacher who is skeptical about the ingredients in vaccines or a retired IT professional and nurse who worries about vaccine hesitancy but still has some lingering misgivings over the U.S. response to the Covid pandemic. For those following the movement, mistrust is often the starting point, and the pandemic was a tipping point. MAHA’s leaders focus on a disparate array of concerns, including some widely shared worries about chemicals in food, and more fringe beliefs disputed by scientists about vaccines. During that time of social distancing, mask wearing, and limits on international travel, one strain of flu B viruses disappeared. In the years since, it was concluded that the B/Yamagata lineage had gone extinct. The component that protected against these viruses was even taken out of flu shots. Flu laboratories around the world remain on the lookout for B/Yam, as it’s known in flu circles, and one now says it has detected the virus. Adam Meijer, who heads the Dutch National Reference Laboratory for Respiratory Viruses, said several independent tests of the a patient’s sample came back positive, though the lab was unable to grow virus, or generate a genetic sequence from the sample. Arnold Monto, a veteran flu epidemiologist at the University of Michigan who pushed to have B/Yamagata removed from U.S. flu shots, said given there is no genetic sequence, it’s too soon to know what to make of this report. Wenqing Zhang, who heads the WHO's global influenza program, also reserved judgment, saying the Dutch lab will be looking for B/Yam viruses in the coming flu season, so if they are circulating “they will be detected.” WHO recommends dropping component of many flu vaccines Helen Branswell By Helen Branswell https://www.statnews.com/2023/09/29/who-recommends-dropping-component-of-many-flu-vaccines/?utm_campaign=morning_rounds&utm_medium=email&_hsenc=p2ANqtz--nun4X90iYfC46QT1wcc1FNvhgyV4AwNzryQuhUTZmKmVRBQpTM1fso7hYSDESKyNaEROEeQfXCCf6ZXMZXRmx6hy7eg&_hsmi=335470246&utm_content=335470246&utm_source=hs_email FDA expert panel endorses idea of removing a component from flu vaccine Helen Branswell By Helen Branswell https://www.statnews.com/2023/10/05/fda-expert-panel-endorses-idea-of-removing-a-component-from-flu-vaccine/?utm_campaign=morning_rounds&utm_medium=email&_hsenc=p2ANqtz--x8IgGRaJYs4OgBVaXt2TFMIfusjmNQOv9imJe7jkaSc8fuK_L8u1egDLDBrIloQRnjTRCcfQjU0BRqM32t6VwpjrECA&_hsmi=335470246&utm_content=335470246&utm_source=hs_email

They had no known contacts with infected animals or poultry, the California Department of Public Health said when the case was first announced. The child, who had only mild symptoms, is recovering. It’s the first confirmed case of H5N1 in a child in the U.S. So far this year 55 confirmed human H5N1 infections have been recorded in seven states. None of the child’s contacts tested positive for the virus, which is good news. But cases where the source of infection cannot be determined are unsettling, raising the specter of undetected transmission of a flu virus to which humans do not have immunity, STAT’s Helen Branswell, who covers infectious diseases, explains in this video. In related perturbing news, California public health officials have detected bird flu virus in a sample of raw milk from Raw Farm in Fresno that was purchased at a retail outlet. (Reminder: A study earlier this year showed that drinking raw milk that contains bird flu viruses could be dangerous.) Helen has the details. https://www.cdc.gov/bird-flu/situation-summary/index.html?utm_campaign=morning_rounds&utm_medium=email&_hsenc=p2ANqtz--ZsXvnNzwHmXN3qOo9kDnGHflTPoCoekeYznu-moEm2uFB5EWKPulDMK-S2jrRSiwMnoW5mJ_6LbeCMomrUqsgBhK0zg&_hsmi=335470246&utm_content=335470246&utm_source=hs_email

Inside UnitedHealth’s strategy to pressure physicians: $10,000 bonuses and a doctor leaderboard Tara Bannow Bob Herman Casey Ross Lizzy Lawrence By Tara Bannow , Bob Herman , Casey Ross , and Lizzy Lawrence

For some types of care, the company’s insurance subsidiary doles out twice the average market price at its own practices. “It’s really a game the way they switch money from their right pocket to their left,” one doctor said. As a reminder, UnitedHealth is the country’s largest health insurer and fourth-largest company of any type by revenue. The company argues in marketing materials that its control of medical providers delivers greater efficiency, better care coordination, and lower costs. But STAT’s findings expose the effects of a deepening conflict of interest: As both a dominant insurance company and a health care provider, UnitedHealth can capture higher profits by paying itself inflated prices for basic checkups, surgeries, and procedures. STAT uncovered the payments in a first-of-its-kind analysis with health analytics company Tribunus Health that examined UnitedHealth’s own data as reported to the federal government. Read more in Part 5. And I urge you to check out the first few parts if you haven’t already. They document how UnitedHealth turned a questionable artery-screening program into a gold mine; it promised a hands-off approach when buying one Connecticut medical group, then upended everything its doctors had built; and its managers use tactics like bonuses and leaderboard rankings to pressure doctors. This series highlights how UnitedHealth Group wields its unrivaled physician empire to boost its profits and expand its influence. Reporters Casey Ross, Lizzy Lawrence, Bob Herman, and Tara Bannow spent months interviewing sources and reviewing documents to deliver this unrivaled look into the biggest and most consequential U.S. health care company. https://www.statnews.com/unitedhealth-group-investigation-health-care-colossus-series/?utm_campaign=morning_rounds&utm_medium=email&_hsenc=p2ANqtz-9ZYtlAdKl48OAo8z2bTlM57PAatp3-q0U8Tlm_MYIOL26s3FzblrV8v8jAzUL1to_ywYmu8AfZlnOAB-kZL-3cbhyqCQ&_hsmi=335470246&utm_content=335470246&utm_source=hs_email

STAT’s Rachel Cohrs Zhang rounded up seven examples from Makary's history of criticizing the agency, including calling out the FDA’s decisions to approve boosters for young people, the pace of its approval for early vaccines, and its remote work policies. Makary is also an executive of the telehealth company Sesame, which connects consumers to physicians who can prescribe compounded weight-loss drugs. If confirmed as FDA commissioner, he would take the lead of the agency as it grapples with high-stakes policy issues that could impact Sesame’s business. Read more from a team of STAT reporters on that potential conflict. While Matt wrote that the CDC is “in big trouble,” under Weldon, he believes that the FDA “might be OK” under Makary. The Food and Drug Administration might be OK. While the FDA pick of Marty Makary seems perhaps the least likely to try to upend the status quo, the presumptive CDC director, Dave Weldon, has been a persistent skeptic about the safety of some vaccinations. That is worrisome for an agency whose most historic victories have come through the use of vaccines.

Family medicine presence on labor and delivery: effect on safety culture and cesarean delivery. November 20, 2024

https://psnet.ahrq.gov/issue/family-medicine-presence-labor-and-delivery-effect-safety-culture-and-cesarean-delivery Family medicine presence on labor and delivery: effect on safety culture and cesarean delivery. VanGompel EW, Singh L, Carlock F, et al. Ann Fam Med. 2024;22(5):375-382. Rural care settings face unique challenges in the provision of safe obstetric care. This study explored whether the presence of family medicine (FM) physicians (who are more likely to attend births in rural settings without access to obstetricians or midwives) impacts safety culture and perinatal outcomes. Researchers surveyed 849 clinicians from 39 hospitals in Iowa and found that FM-only hospitals had a lower risk of cesarean deliveries, more support for vaginal birth, and stronger safety culture compared to hospitals with both FM physicians and obstetricians.

Wahlstedt E, Levy BE, Scott E, et al. J Patient Saf. 2024;Epub Oct 18. The root cause analysis or RCA2 framework is a standardized approach to identify patient safety incidents based on their potential harm and frequency. In this study, researchers used an RCA2 algorithm to identify high-risk events for root cause analysis. Among 104 cases identified from the VA health system, the algorithm recommended nearly 57% for RCA. In comparison, only 17% of cases met the current institutional selection process, missing 45 potential high-frequency, high-harm events.

Nurse burnout and patient safety, satisfaction, and quality of care: a systematic review and meta-analysis. November 20, 2024

https://psnet.ahrq.gov/issue/nurse-burnout-and-patient-safety-satisfaction-and-quality-care-systematic-review-and-meta Nurse burnout and patient safety, satisfaction, and quality of care: a systematic review and meta-analysis. Li LZ, Yang P, Singer SJ, et al. JAMA Netw Open. 2024;7(11):e2443059. Nurse burnout increased during the pandemic and remains prevalent. This review sought to determine the association between nurse burnout and patient safety, patient satisfaction, and quality of care. Nurse burnout increased over time and was negatively associated with these factors. The association between the subcomponents of burnout (e.g., safety culture, emotional exhaustion, and depersonalization) and lower health care quality, safety and patient satisfaction is also presented.

Experiences with diagnostic delay among underserved racial and ethnic patients: a systematic review of the qualitative literature. November 20, 2024

https://psnet.ahrq.gov/issue/experiences-diagnostic-delay-among-underserved-racial-and-ethnic-patients-systematic-review Experiences with diagnostic delay among underserved racial and ethnic patients: a systematic review of the qualitative literature. Faugno E, Galbraith AA, Walsh KE, et al. BMJ Qual Saf. 2024;Epub Nov 4. Historically, underserved racial and ethnic populations have been disproportionately affected by delayed diagnosis. In this review of quantitative studies conducted in the US, patients and caregivers in underserved communities reported several factors contributing to delayed diagnosis. Socioeconomic and sociocultural factors included healthcare avoidance, stigma, and distrust in the healthcare system. Health system factors included poor organizational health literacy and provider-related factors such as cognitive biases. There was a lack of research on diagnosis of chronic conditions, and no studies focused on Asian Americans and Pacific Islanders.
