Q&A: Climate crisis as health crisis
Last month, Hurricane Helene caused a shortage of IV fluids in hospitals across the country after the storm forced a facility in North Carolina to close. Tampa Bay General Hospital puts up a literal flood wall during storms to keep patients safe. “Climate change is here, and we feel it through its impact on our health,” physician and non-profit CEO Vanessa Kerry said in a Q&A with STAT’s Nick St. Fleur. Nick spoke to Kerry at the Milken Future of Health Summit earlier this month about climate change, global health, and the incoming Trump administration.

Antibiotics are often given to preterm and vulnerable newborns before infections are confirmed in neonatal intensive care units (NICUs). In an article published in the Journal of Intensive Care Medicine, AHRQ-supported scholars reviewed general principles of antibiotic stewardship, a careful approach to reducing unnecessary antibiotic use, and how it can be applied to NICUs to avoid adverse effects and limit antibiotic resistance. They summarized current antibiotic use metrics and reviewed evidence-based infection prevention practices in the NICU. They also provided current recommendations for practical antibiotic use in the NICU, summarized the usefulness of infection biomarkers and highlighted specific threats in the NICU. They concluded that by implementing these practices, NICUs can better protect newborns from risks associated with overusing antibiotics, improve patient outcomes and help combat the global issue of antibiotic resistance.

Another resource for supporting youth experiencing homelessness, and youth in general, is the Family & Youth Services Bureau (FYSB), part of the U.S. Department of Health and Human Services’ Administration for Children and Families. FYSB supports organizations that work to promote youth well-being, prevent and end youth homelessness, and support a holistic adolescent approach by fostering collaborative partnerships across communities; leading in partnership with youth and young adults; promoting diversity, equity, inclusion, and accessibility; and supporting data-driven practices. Among FYSB’s programs is the Runaway and Homeless Youth Program. FYSB also funds and partners in providing services with the National Runaway Safeline (NRS), which offers a crisis hotline and online services 24 hours a day, 365 days a year to youth at risk of running away, those who have run away or are homeless, and their families. NRS offers services including crisis intervention, information and referrals, three-way conference calls, and more. WASHINGTON — The Biden administration is proposing a major expansion of how Medicare and Medicaid cover the blockbuster weight loss drugs Wegovy and Zepbound, the White House announced Tuesday.
Medicare currently covers the medications made by Novo Nordisk and Eli Lilly to treat diabetes and heart disease, but not for obesity alone because it’s prohibited by law from doing so. Given the price tag, a bill in Congress to extend coverage has stalled for years.
The White House estimates the new proposal would provide coverage to an estimated 3.4 million older adults in the Medicare program, and 4 million people in Medicaid. The proposal would go into effect in 2026, but it's unclear whether the incoming Trump administration would support such a measure, or would work to roll it back.

How Trump travel bans could affect scientists
Within a week of taking office in 2017, Donald Trump signed an executive order prohibiting people from seven majority-Muslim countries from entering the U.S. The impacts were felt almost immediately in the science community, as researchers who’d planned to work here or present at scientific conferences suddenly weren’t able to enter the country. With his reelection, members of the life sciences community are concerned that Trump will make good on his campaign promise to reinstate and expand travel restrictions put in place during his first term.

With the election of Donald Trump to the White House — and his decision to nominate Robert F. Kennedy Jr. to head the Department of Health and Human Services — the MAHA movement has taken center stage. That, however, doesn’t mean that all its adherents fit under a neat political umbrella.

For those following the movement, mistrust is often the starting point, and the pandemic was a tipping point. MAHA’s leaders focus on a disparate array of concerns, including some widely shared worries about chemicals in food, and more fringe beliefs disputed by scientists about vaccines. This flu lineage might have come back from extinction
From one flu mystery to another, STAT’s Helen Branswell also brings us this report:
In the first year or so of the Covid-19 pandemic, there was very little influenza activity globally. During that time of social distancing, mask wearing, and limits on international travel, one strain of flu B viruses disappeared. In the years since, it was concluded that the B/Yamagata lineage had gone extinct. The component that protected against these viruses was even taken out of flu shots.
Flu laboratories around the world remain on the lookout for B/Yam, as it’s known in flu circles, and one now says it has detected the virus. Adam Meijer, who heads the Dutch National Reference Laboratory for Respiratory Viruses, said several independent tests of the a patient’s sample came back positive, though the lab was unable to grow virus, or generate a genetic sequence from the sample.
Arnold Monto, a veteran flu epidemiologist at the University of Michigan who pushed to have B/Yamagata removed from U.S. flu shots, said given there is no genetic sequence, it’s too soon to know what to make of this report. Wenqing Zhang, who heads the WHO's global influenza program, also reserved judgment, saying the Dutch lab will be looking for B/Yam viruses in the coming flu season, so if they are circulating “they will be detected.”

U.S. confirms first H5N1 case in a child
The CDC confirmed on Friday that a California child was infected with H5N1 bird flu. As with a case in Missouri that STAT’s Helen Branswell reported on earlier this fall, the source of the infection is not known. The child, who lives in Alameda County southeast of Oakland, does not live on or near a farm. They had no known contacts with infected animals or poultry, the California Department of Public Health said when the case was first announced. The child, who had only mild symptoms, is recovering. It’s the first confirmed case of H5N1 in a child in the U.S. So far this year 55 confirmed human H5N1 infections have been recorded in seven states.
None of the child’s contacts tested positive for the virus, which is good news. But cases where the source of infection cannot be determined are unsettling, raising the specter of undetected transmission of a flu virus to which humans do not have immunity, STAT’s Helen Branswell, who covers infectious diseases, explains in this video.

UnitedHealth Group is paying many of its own physician practices significantly more than it pays other doctor groups in the same markets for similar services, STAT reporters reveal in Part 5 of the series Health Care’s Colossus. For some types of care, the company’s insurance subsidiary doles out twice the average market price at its own practices. “It’s really a game the way they switch money from their right pocket to their left,” one doctor said.
As a reminder, UnitedHealth is the country’s largest health insurer and fourth-largest company of any type by revenue. The company argues in marketing materials that its control of medical providers delivers greater efficiency, better care coordination, and lower costs. But STAT’s findings expose the effects of a deepening conflict of interest: As both a dominant insurance company and a health care provider, UnitedHealth can capture higher profits by paying itself inflated prices for basic checkups, surgeries, and procedures.

Rural care settings face unique challenges in the provision of safe obstetric care. This study explored whether the presence of family medicine (FM) physicians (who are more likely to attend births in rural settings without access to obstetricians or midwives) impacts safety culture and perinatal outcomes. Researchers surveyed 849 clinicians from 39 hospitals in Iowa and found that FM-only hospitals had a lower risk of cesarean deliveries, more support for vaginal birth, and stronger safety culture compared to hospitals with both FM physicians and obstetricians.

The root cause analysis or RCA2 framework is a standardized approach to identify patient safety incidents based on their potential harm and frequency. In this study, researchers used an RCA2 algorithm to identify high-risk events for root cause analysis. Among 104 cases identified from the VA health system, the algorithm recommended nearly 57% for RCA. In comparison, only 17% of cases met the current institutional selection process, missing 45 potential high-frequency, high-harm events.

Historically, underserved racial and ethnic populations have been disproportionately affected by delayed diagnosis. In this review of quantitative studies conducted in the US, patients and caregivers in underserved communities reported several factors contributing to delayed diagnosis. Socioeconomic and sociocultural factors included healthcare avoidance, stigma, and distrust in the healthcare system. Health system factors included poor organizational health literacy and provider-related factors such as cognitive biases. There was a lack of research on diagnosis of chronic conditions, and no studies focused on Asian Americans and Pacific Islanders.
