Executive Summary Effective approaches exist to prevent and treat mental illness and to promote mental health but most people who could benefit from evidence-based interventions (policies, programmes, and individual-level practices or services) do not receive them. Too often, research produces interventions and implementation strategies that are difficult to scale owing to misalignment with the political, cultural, policy, system, community, provider, and individual realities of real-world settings. This Commission considers strategies for transforming how research is done to produce more actionable evidence. It examines how to integrate research and real-world implementation; centre equity in mental health intervention and implementation research; apply a complexity science lens to mental health research; expand designs beyond the randomised clinical trial; and value transdisciplinarity across endeavours. United States (consolidated with United States v. Idaho). A majority of the justices decided that the Court should not have taken the case at this time and, because of that, there is no ruling on the core questions presented and the case returns to lower courts for further proceedings. The decision also reinstates a District Court’s order blocking enforcement of the Idaho abortion ban where it conflicts with EMTALA, the federal law requiring hospitals to provide stabilizing treatment to patients who present to their emergency rooms. Idaho – like 5 other states – has an abortion ban that includes an exception to save the life of the pregnant person, but no exception for preserving health. Today’s decision does not affect enforcement of bans in any other states. This decision means the Court did not rule on the merits on any of the abortion-related cases this term, a topic of high significance to many voters ahead of the 2024 presidential elections.

Although a cure for hepatitis C is available, many people with the potentially deadly disease are not aware that they have it and do not receive treatment. This is partly because hepatitis C testing has been a lengthy process before results are available and requires follow-up appointments. The use of real-world data (RWD) to improve the efficiency of drug and biologic development programs that rely on traditional clinical trials is well established and encouraged by FDA. This is the first annual report describing submissions to CBER and CDER that contain analyses of RWD to generate RWE intended to support regulatory decision-making about a drug or biological product’s effectiveness or safety. The report meets an agency commitment under the Prescription Drug User Fee Act (PDUFA VII).

Enhancing diversity within clinical studies not only facilitates broader applicability of results across a broad spectrum of patient populations, but also enhances understanding of the disease or medical product under study, thus providing valuable insights to inform the safe and effective use of the medical product among patients. “Participants in clinical trials should be representative of the patients who will use the medical products,” said FDA Commissioner Robert M.
