This article describes an 18-month clinical learning collaborative among 16 sites intended to increase resident and fellow participation in patient safety event investigations. This survey of 52 obstetrics and gynecology departments at academic medical centers found that few departments provided faculty with protected time or financial support for quality improvement activities, and only 5% of departments included a patient representative on the quality committee.

Improving diagnosis in pediatrics is an ongoing patient safety focus. This retrospective study included 129 pediatric emergency transfer cases and examined the relationship between missed opportunity for improvement in diagnosis (MOID; determined using SaferDx) and patient outcomes. Researchers found that MOID occurred in 29% of emergency transfer cases and it was associated with higher risk of mortality and longer post-transfer length of stay.v
Healthcare fragmentation, multimorbidity, potentially inappropriate medication, and mortality: a Danish nationwide cohort study.

System weaknesses (e.g., resource availability, deficiencies in care coordination) threaten patient safety. This population-based cohort study including 4.7 million Danish adults who interacted with primary or hospital care in 2018, found that indicators of care fragmentation (e.g., higher numbers of involved clinicians, more transitions between providers) increased with patient morbidity level. The researchers found that higher levels of care fragmentation were associated with adverse outcomes, including potentially inappropriate prescribing and mortality.

This survey of 118 nursing homes in Switzerland found that organizations employ a variety of electronic health record (EHR)-based tools to support medication safety, such as standardized medication lists, alerts for potentially inappropriate prescribing, or electronic data exchanges with community pharmacies or outside physicians.
Estimating breast cancer overdiagnosis after screening mammography among older women in the United States.

Overdiagnosis can result in overtreatment, physical harm, and emotional distress. Using SEER-Medicare data, researchers examined breast cancer overdiagnosis by comparing cancer incidence among women who discontinued mammography screening after age 70 compared to women who continued to receive screening mammograms. Findings suggest that breast cancer may be potentially overdiagnosed among 31% of women aged 70 to 74 years, 47% of women aged 75 to 84 years, and 54% of women aged 85 and older who continue to receive screening mammograms.

This article describes ACTMed (ACTivating primary care for MEDicine safety), a cluster randomized trial set in Australia which intends to improve medication safety in primary care settings. The ACTMed intervention will use health information technology (e.g., clinical indicator algorithms), guideline-based clinical recommendations, shared decision-making, and financial incentives to reduce serious medication-related harm, medication-related hospitalizations, and death.

This systematic review including 44 articles examined the diagnostic agreement between teledermatology and face-to-face consults. Hospital-acquired venous thromboembolism (VTE) remains a significant source of preventable patient harm. This study of 4,252 high-risk general surgery patients found that only one-third received care in compliance with VTE prophylaxis guidelines. Patients receiving guideline-compliant care experienced shorter lengths of stay (LOS), fewer blood transfusions, and decreased odds of having a VTE, emphasizing the importance of initiating VTE chemoprophylaxis in high-risk general surgery patients.

The National Institute of Mental Health’s (NIMH) Global Mental Health Team is hosting a series of listening sessions to hear the perspectives and experiences of individuals involved in mental health research across a range of career stages and roles, including investigators, research staff, grants management and administrative staff, and policymakers.
We are particularly interested in hearing from individuals who are from low- and middle-income countries (LMICs) or who are currently working at LMIC institutions, as well as from those in under-resourced communities.
During these sessions, the Global Mental Health Team hopes to gather insights and experiences about various aspects of mental health research, such as institutional capacity building, training, building independent careers, mentorship, applying for and managing grants, forming partnerships and collaborations, investing in long-term projects, and ensuring the relevance of mental health research in real-world practice.
Sessions will be held between mid-September and late October. Each session will offer 9:00 a.m. – 10:30 a.m. ET and 10:00 p.m. – 11:30 p.m. ET time slots to accommodate time differences.

Individual variability in drug response efficacy or safety is becoming an increasing problem globally, given the growing population of older adults. Reasons for this variability include genomic factors, an area of study called pharmacogenomics. With genotyping technologies now widely available and decreasing in cost, implementing pharmacogenomics into clinical practice is widely viewed as an initial step in mainstreaming genomic medicine. The benefits of pharmacogenomic testing before starting drug therapy has been well documented for several single gene-drug combinations. In addition, the clinical utility of a pre-emptive genotyping strategy using a pharmacogenetic panel is being rigorously assessed. However, major challenges of implementation lie at the point of integration into healthcare systems, including the modification of clinical pathways and a large knowledge gap in pharmacogenomics in the healthcare workforce. Pharmacogenomics can also be used in a broader sense for drug discovery and development, with increasing evidence suggesting that genomically defined targets have an increased success rate during clinical development. Our speaker will review recent developments in the field of pharmacogenomics and the ongoing challenges in equitable translation of discoveries into clinical practice and public health programs.

Many healthcare professionals experienced adverse emotional and psychological outcomes during the COVID-19 pandemic. This survey of 665 health system pharmacists found that pharmacists working in settings with higher levels of workplace wellness support were less likely to experience depression, anxiety, or burnout, and report higher levels of professional quality of life during the COVID-19 pandemic.

Fostering a culture of safety is a core patient safety objective. This survey of 425 radiologic technologists explored differences in radiation safety culture between staff technologists and those in leadership roles. Findings identified several positional hierarchical imbalances in perceived determinants of safety culture, which could hinder efforts to establish a just culture and a positive organizational radiation safety culture.

The goal of this study was to determine whether treatment approaches (e.g., palliative care to aggressive treatment) were associated with clinician cognitive biases in acute care settings for patients with advanced dementia and comorbidities. Representativeness, availability, and anchoring biases were associated with treatment approach in this hypothetical patient case; moral characteristics of the clinician were not associated with treatment approach.

Understanding causes of wrong-patient order entry (WPOE) can help develop interventions to reduce those medication errors. This review summarizes how organizations and providers identify WPOE, what data are being captured, and causes. The most common organizational detection method is the retract-and-reorder method whereby a medication order is cancelled then reordered on a different patient within a specified period of time. There was minimal data on how providers detect their own WPOE errors. Technology and physician workload were identified as contributors to WPOE.

Rapid response teams (RRT) are activated, typically by nurses, when a patient demonstrates signs of imminent clinical deterioration, in order to prevent death or transfer to the intensive care unit (ICU). This study asks ICU managers about their perceptions of RRT beyond the stated goal of preventing patient deterioration. They describe the RRT as providing valuable education for new nurses and physicians and enhancing cohesion between the ICU and other wards. However, nurse managers stated they wanted more data and feedback from executive leadership.

Reducing central line-associated bloodstream infections (CLABSI) is an important patient safety improvement target. This study examined the relationship between compliance with evidence-based CLABSI guideline bundles and CLABSI rates in 159 hospitals. Between 2011 and 2021, researchers found that adherence to bundle guidelines was associated with a significant reduction in CLABSI rate.

Quantitative studies have demonstrated that emergency department (ED) overcrowding and patient boarding contribute to adverse events. This study includes both qualitative and quantitative methods to understand how ED clinicians view the impact of boarding on their own well-being as well as patient safety. Key themes include clinician dissatisfaction and burnout, and high rates of verbal and physical abuse from boarding patients. Possible solutions included improved standardization of care, proactive planning, and culture change hospital-wide.

Diagnostic errors are common and have many contributing factors. This study analyzed more than 100 serious adverse event (SAE) reports in acute care using four investigation methods (e.g., Diagnostic Error Evaluation Research (DEER) taxonomy, Safer Dx Instrument) to identify common contributing factors. Transitions of care were particularly vulnerable to SAE, often due to incomplete communication between departments. Diagnostic errors occurred most often in the testing, assessment, and follow-up phases, with human factors as the most common contributing factor. Using multiple investigative methods supports more targeted interventions in each phase of diagnosis.

Treatment of Stage I-III Squamous Cell Anal Cancer: This is an new protocol for a systematic review that will summarize the state of the evidence on treatment of stage I-III squamous cell carcinoma of the anal canal and anal margin.
This systematic review will assess the effectiveness and harms of treatment for stages I-III squamous cell anal cancer. This review, funded by the Patient-Centered Outcomes Research Institute, will be used by the American Society of Clinical Oncology and the American Society of Radiation Oncology to update clinical practice guidelines.

Diagnosis and Management of Obsessive Compulsive Disorders in Children: The systematic review will summarize the findings from 1. Studies related to the accuracy of assessment tools compared to reference standard methods to identify OCD in symptomatic youth and 2. Studies of psychological and/or pharmacological treatments of OCD.
This comparative effectiveness review will inform a planned update of the 2012 American Academy of Child and Adolescent Psychiatry (AACAP) Practice Parameter. AACAP nominated this topic to the Patient-Centered Outcomes Research Institute, which contracted with the Agency for Healthcare Research and Quality to conduct the review. Specifically, the systematic review will summarize the findings from 1. Studies related to the accuracy of assessment tools compared to reference standard methods to identify OCD in symptomatic youth and 2. Studies of psychological and/or pharmacological treatments of OCD. The intended audience includes guideline developers, child psychiatrists and psychologists, pediatricians, family physicians, advanced practice providers, parents, and patients.

Join us on September 12 and 13, 2023, to explore good clinical practice considerations for clinical trials. The U.S. Food and Drug Administration, in collaboration with the Reagan-Udall Foundation for the FDA, will convene international experts in the conduct of clinical trials with pragmatic or decentralized features. Clinical trials with streamlined protocols that are embedded in existing health care infrastructure (often referred to as pragmatic trials), and trials with decentralized processes have the potential to generate evidence more efficiently while minimizing the burden on participants.

Insomnia is common in schizophrenia. Insomnia has been associated with suicidal ideation and behavior, as well as greater severity of psychopathology, in schizophrenia. This presentation assesses the evidence regarding these associations and discusses the use of pupillometry as a novel marker of suicide risk in patients with psychiatric disorders.
