Upcoming CHART Model Office Hour: Community Transformation Track

Join us on October 27th for Community Health Access and Rural Transformation (CHART) Model’s Office Hour on the Community Transformation Track
The CHART Model will test whether upfront investments, predictable capitated payments, and operational and regulatory flexibilities will enable rural health care providers to improve access to high quality care while reducing health care costs.
The CHART Model is offering two track options for participation: the Community Transformation Track and the Accountable Care Organization (ACO) Transformation Track. CMS announced the Notice of Funding Opportunity (NOFO) for the CHART Model’s Community Transformation Track in September 2020. Applications for the Community Transformation Track are due by February 16, 2021. The Request for Application (RFA) for the ACO Transformation Track will be available in early 2021.
CHART Model Office Hour: Tuesday, October 27, 2:00-3:00 pm ET
On October 27, 2020, the Centers for Medicare & Medicaid Services (CMS) will host an office hour, on the CHART Model’s Community Transformation Track. During this event, the CHART Model team will provide an overview of and answer your frequently asked questions on the CHART Community Transformation Track. The CHART Model team will answer questions on key topics such as eligibility, payment, benefit enhancements, program overlap, and Medicaid participation. This event will focus specifically on the Community Transformation Track. All interested stakeholders are encouraged to submit questions in advance through their webinar registration or by sending an email to CHARTModel@cms.hhs.gov.
