Linking Clinical Practices and Community Organizations to Improve Care
Linking Clinical Practices and Community Organizations for Prevention Background and Introduction
The Agency for Healthcare Research and Quality has established a goal of improving care along the continuum from primary prevention to chronic care management. This includes health promotion, disease prevention, health maintenance, and the diagnosis and treatment of chronic illnesses. The Agency seeks to explore how partnerships among primary care and medical specialty providers, the community, and public health organizations are developed, strengthened, and sustained to improve care and meet the needs of patients and families.
