CMS adds to ACO, ESCO resources with a video and three new case studies on patient engagement
CMS is announcing the release of a video that describes two Medicare ACO models and features highlights from interviews with Next Generation ACO and End-Stage Renal Disease Seamless Care Organization (ESCO) representatives. These representatives describe how ACOs and ESCOs work to improve care for their beneficiaries and how patients benefit from value-based care.
Adding to existing resources, CMS is releasing three new case studies that describe how ESCOs and ACOs support and engage beneficiaries in their care.
- Providing nonclinical support for beneficiaries with ESRD through a patient navigator program (Atlantic Dialysis Management Services case study)
- Maintaining patient advisory committees to improve care delivery for patients with ESRD (Centers for Dialysis Care)
- Leveraging health navigators to identify and resolve care gaps (Keystone ACO)
Each case study describes how an ESCO or ACO implemented a program or initiative, as well as early results and lessons learned. These case studies join a collection of resources for organizations participating in value-based care models, including toolkits on beneficiary engagement and care coordination, and over a dozen case studies that describe a variety of ACO strategies to improve care delivery.
