Use of failure mode and effects analysis for proactive identification of communication and handoff failures from organ procurement to transplantation.

Failure mode and effects analysis was used to identify vulnerable handoff and communication processes in organ procurement and transplantation. A communication error led to the widely publicized death of a lung transplant patient at Duke University Medical Center in 2003.

Leadership WalkRound films- Patient Safety First Campaign

Leadership WalkRound Films.

This site presents four short films that highlight National Health Service implementation of WalkRounds initiatives, which enable managers and other health care leadership to learn about potential safety problems by engaging with frontline staff.

Leadership WalkRound filmsIn conjunction with the NHS Institute Safer Care team we have developed four short films that highlight the process of implementing Leadership Safety WalkRounds in three NHS Trusts in England.

The first film provides an overview of Leadership WalkRounds and we hear from NHS staff about their tangible impact at the front-line. Members of the Patient Safety First core team also provide tips and advice about implementing Leadership WalkRounds based on their own recent experience. In the second and third films we explore the perspectives of Executives and Patient Safety Managers who discuss their experiences to date. The final film is a short version of the overview.

These films are intended to help raise awareness about the potential impact of Leadership WalkRounds as practical and visible ways of developing an organisations safety culture. They are not intended to provide comprehensive information or to be an exhaustive training tool and we recommend that they are used in conjunction with the Patient Safety First Leadership WalkRound guide.

Novedades del próximo congreso y jornadas:
1) El Congreso y Jornadas 2009 cuenta con: 5 cursos pre-congreso (requieren inscripción previa en la SAM: http://congreso.sam.org.ar/contacto.php), 3 cursos intracongreso, 5 talleres docentes, 4 simposios y workshops, 2 conferencias magistrales, 3 ateneos aátomo-clinicos y 27 mesas redondas, a cargo de más de 400 participantes. Ver programa (http://www.sam.org.ar/congreso_nacional_medicina/2009/Temario.pdf).
2) Se presentarán más de 740 trabajos científicos, provenientes de casi todas las provincias argentinas y además de países latinoamericanos como Uruguay, Paraguay, Bolivia, Colombia, Ecuador. Esta actividad compromete a más de 90 revisores y al menos unos 150 comentadores en la presentación de dichos trabajos..
3) El día 4 de noviembre se programó una actividad de encuentro entre residentes de todo el ámbito nacional y del exterior para generar un nuevo espacio de convocatoria INTERNACIONAL centrado en la producción científica.
4) Aconsejamos varias cosas con el fin de moverse dentro del congreso con la mayor celeridad posible:
a) tener impreso el plano del congreso (http://congreso.sam.org.ar/Plano_congreso.pdf).
b) hacer los trámites de registración, adonde se muestran en el plano: autoridades, disertantes e invitados en la secretaría de adelante; presentadores, autores y asistentes que deben incribirse secretaría del fondo a la derecha y becados igual secrtaría pero a la izquierda.
c) usar las escaleras o ascensores que los lleven a los salones según mapa, ya que circular entre los salones esta prohido mientras duren las actividades académicas.
d) tendremos un sistema telefónicos de celular que permitirá saber que actividades hay en los salones (con el contido resumido del programa científico y de las presentaciones de trabajos).
5) Para la presentación de trabajos ver día, hora, lugar y ejemplos de presentacón en la página de la SAM o en la página del Congreso (http://congreso.sam.org.ar/)
6) Cena y fiesta de cierre, ir comprando las entradas a módicos precios, en las secretarias para tal fin.
