Experience feedback committees: a way of implementing a root cause analysis practice in hospital medical departments.

This study describes how a French hospital implemented a multidisciplinary root cause analysis infrastructure called experience feedback committees. Over one year, teams of physicians, nurses, and other health care professionals in each department reviewed 1707 adverse events and selected 206 corrective actions. An Annual Perspective discussed the limitations of root cause analysis and how this tool can be improved.
