ISMP Medication Errors Reporting Program.
The Institute for Safe Medication Practices (ISMP) and Quantros collaborate to administer this national reporting program, which collects confidential reports of medication errors and near misses directly from practitioners. Information is forwarded to the U.S. Food and Drug Administration and product manufacturers. The program was formally co-administered by United States Pharmacopeia (USP).
