A transparent and respectful hospital culture is the foundation for improving working conditions to reduce preventable harm. This commentary describes one health system's multimodal strategy to enhance staff understanding of their roles and responsibilities regarding patient safety. The program resulted in substantial improvements and had a modest financial impact on the organization.
