Web Seminar Now Available - Two ACL Dementia Grantee Developed Products: Virginia’s Collaborative Care Coordination Model, and Eddy Alzheimer’s Services Training on Dementia Care for People with IDD

ACL dementia grantees often develop models and products used not only by their organizations, but also can be adopted by others working in the dementia field. This web seminar will feature two grantee programs: a model of coordinated dementia care, and dementia training for professional caregivers of people with intellectual and developmental disabilities.

The coordinated care model was developed and piloted through a relationship between the University of Virginia's Memory and Aging Care Clinic and the Jefferson Area Board for Aging, a local Area Agency on Aging. The presenter will detail the importance of coordinated care for people living with dementia and their care partners, and will describe the program, its outcomes, lessons learned and challenges. Links to pertinent materials will be provided in the webinar. 

The dementia training was designed specifically for professional caregivers of people with intellectual and developmental disabilities. The presenter will describe the tools and resources used, and where to find them. A discussion of challenges, successes and lessons learned will help participants to consider the practical application of a similar curriculum in their practice setting.

Presenters:

Jennifer Harvey is a social worker and Certified Dementia Practitioner in New York’s Capital Region, providing education to professional and family caregivers of persons living with dementia.

George Worthington is the Dementia Services Coordinator with the Virginia Department for Aging and Rehabilitative Services, where he supports the Virginia Alzheimer’s Disease and Related Disorders Commission, coordinates efforts to implement the Dementia State Plan and is the co-lead of the Dementia Friendly Virginia initiative.
