This article describes the pilot implementation of the Geriatric High-Risk Evaluation and Liaison Program – Transitional Care (GHELP-TC) which aims to improve care transitions between acute care to skilled nursing and skilled nursing to home for older veterans. This pilot evaluation identified 79 medication errors and 80 appointment errors among 90 enrolled veterans. A planned expanded implementation will include registered nurses (RNs) to improve communication and accountability.
