https://cahpsdatabase.ahrq.gov/HPDSS/login.aspx Voluntary data submission for AHRQ’s Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Health Plan Survey Database will be open June 1-26 to all CAHPS 5.1/5.1H Health Plan Survey users who administered the survey June 28, 2025-June 26, 2026.
