June 1 – July 17: CAHPS^® Health Plan Survey Database Submission Extension

CAHPS^® 5.0/5.0H Health Plan Survey users who administered the survey July 2019 – July 2020 may submit their data to the CAHPS Database June 1 – July 17.

Data collected will be accepted for the following 5.0 and 5.0H survey versions:

• Adult Medicaid
• Child Medicaid or Children’s Health Insurance Program (CHIP) without the Children with Chronic Conditions Item Set
• Child Medicaid or CHIP with the Children with Chronic Conditions Item Set
