ACL Announces 2020 CDSME and Falls Prevention Grantee Cohorts
ACL is pleased to announce the 2020 cohorts of Chronic Disease Self-Management Education (CDSME) and Falls Prevention grantees. The project period for these grants is August 1, 2020 – July 31, 2023.
The new CDSME grantees will reach an estimated 21,000 participants through evidence-based CDSME and self-management support programs. The cooperative agreements are intended to increase the number of CDSME program participants, while concurrently enhancing the sustainability of these proven programs.
The new Falls Prevention grantees will reach over 29,000 older adults with evidence-based programs to reduce the risk of falling and safeguard their independence. Grantees are tasked with delivering these programs, while also increasing their sustainability beyond the life of the grant.
