This three-page case study details how a long-established hospital-community collaboration of Chicago's Loyola Medicine with Proviso Partners for Health (PP4H) has greatly aided efforts to address the food insecurity needs of its vulnerable populations affected by health disparities, which are exasperated during the COVID-19 pandemic.
