Researchers used simulated medication orders involving potential adverse events or deaths to measure the safety performance of electronic health record (EHR) systems. They evaluated how well the systems identified errors as well as the mitigating effects of computerized physician order entry and clinical decision support tools. Safety performance increased moderately over the 10-year study period, but there was considerable variation based on the EHR vendor and whether decision support was basic or more complex. Safety risks persisted despite EHR implementation.
