The impact of outpatient adverse drug events (ADEs) on patients and health systems is substantial. ADEs result in more than 3.5 million physician office visits and 1 million U.S. emergency room visits annually. To minimize harm to patients from ADEs after newly initiated medications, the Brigham and Women’s Hospital system implemented and evaluated an automated telephone surveillance system with an option to transfer to a live pharmacist to screen for drug-related symptoms.
