2016 NEDS and KID Released!

The Agency for Healthcare Research and Quality (AHRQ) has released the 2016 Kids’ Inpatient Database (KID) and Nationwide Emergency Department Sample (NEDS).

The KID is the largest publicly available all-payer pediatric inpatient health care database in the United States, yielding national estimates of hospital inpatient stays for children. The KID is a sample of pediatric discharges from all community, nonrehabilitation hospitals in States participating in HCUP. Pediatric discharges are defined as all discharges in which the patient was 20 years old or younger at admission. KID data are available every 3 years from 1997 through 2012—and now are available for 2016—allowing researchers to analyze trends over time. The number of States in the KID has grown from 22 in the first year (1997) to 46 plus the District of Columbia in 2016.

The NEDS is the largest publicly available all-payer emergency department (ED) database in the United States. It was created to enable analyses of ED utilization patterns and to support public health professionals, administrators, policymakers, and clinicians in their decision making regarding this critical source of care. Constructed using records from both the HCUP State Emergency Department Databases (SEDD) and the State Inpatient Databases (SID), the 2016 NEDS contains data from 33 million ED visits in 36 States and the District of Columbia—approximating a 20 percent stratified sample of visits to U.S. hospital-owned EDs. Weights are provided to calculate national estimates pertaining to the roughly 144 million ED visits that took place in 2016. The NEDS provides information on patients admitted to the hospital through the ED and on patients treated and released or transferred to another hospital from the ED.

HCUP Fast Stats Data Update!

The HCUP Fast Stats Interactive U.S. Map of Opioid-Related Hospital Use has been updated to include maps from 2006–2016. Users can explore how opioid-related inpatient stays and ED visits have changed over time by toggling the map through any year from 2006 through the most current data year of 2016.

Recently, this online tool was updated to add new information to two existing topics:

Opioid-Related Hospital Use features new inpatient and ED data. Since the last update in April of this year, this section has included additional 2016 inpatient data (23 States), 2016 ED data (19 States), 2017 inpatient data (20 States), and 2017 ED data (3 States).
State Trends in Hospital Use by Payer update adds to the inpatient portion of this topic 2015 inpatient data for 1 State, 2016 inpatient data for 5 States, 2017 inpatient data for 15 States, and 2018 inpatient data for 4 States. This update also adds to the ED portion of this topic 2015 ED data for one State, 2016 ED data for nine States, and 2017 ED data for five States.

Updated HCUP Sample Design Tutorial Available

Changes to the HCUP Sample Design Tutorial recently have been implemented to account for HCUP updates. This tutorial provides an in-depth explanation of the sampling strategy for the National Inpatient Sample (NIS), the KID, and the NEDS. Specifically, the module helps users achieve three important goals:

Understand the sample designs for three of the HCUP nationwide databases
Understand how the sample designs can influence which database is best for your research
Avoid common errors that result from misunderstanding the sample design

The Nationwide Readmissions Database (NRD) is another HCUP nationwide database but is not covered in the HCUP Sample Design Tutorial. For more information on the NRD sample design, please visit the Nationwide Readmissions Database (NRD) Tutorial.

Since September 2018, the following State databases have been released:

New HCUP Statistical Briefs Posted on HCUP-US

New HCUP Methods Series Reports Now Available

Since September 2018, the following Methods Series Reports have been released:

Method Series Report #2018-02: An Examination of CHIP and Medicaid Expected Payer Coding in HCUP Databases expands on a 2014 report #2014-03: User Guide: An Examination of Expected Payer Coding in HCUP Databases, in two key ways. First, it focuses specifically on Medicaid and the Children’s Health Insurance Program (CHIP) and includes an analysis of CHIP-specific HCUP expected payer codes (available for a small subset of States). Second, the current report uses more recent HCUP data and enrollment data than did the previous report.
Method Series Report #2018-03: Population Denominator Data Sources and Data for Use With the HCUP Databases (Updated With 2017 Population Data) identifies relevant sources of population data through the 2017 data year that can be used with the HCUP databases to calculate rates of hospital care events per population.
Method Series Report #2018-04: User Guide: An Examination of Expected Payer Coding in HCUP Databases (Updated for 2016 HCUP Data) informs HCUP data users about expected payer codes by (1) presenting detailed information about the expected payer codes collected by HCUP States, (2) suggesting how these payer codes can be used for research purposes, and (3) examining the extent to which HCUP data capture discharges covered by these payers in comparison with other national data sources.

Publications Using HCUP Data

Gupta A, Nizamuddin J, Elmofty D, et al. Opioid abuse or dependence increases 30-day readmission rates after major operating room procedures: a national readmissions database study. Anesthesiology. 2018;128(5):880-90.

This study used the 2013–2014 Nationwide Readmissions Database (NRD) to explore the extent to which opioid abuse or dependence affects readmission rates and health care utilization for patients who underwent a major operating room procedure. The article abstract is available via PubMed.

McCutcheon BA, Hirshman BR, Gabel BC, et al. Impact of neurosurgeon specialization on patient outcomes for intracranial and spinal surgery: a retrospective analysis of the Nationwide Inpatient Sample 1998-2009. J Neurosurg. 2018;128(5):1578-88.

This study used the Nationwide Inpatient Sample (NIS) to examine the trend in modern subspecialization of neurosurgery practices and related improvements in patient morbidity and mortality rates. The article abstract is available via PubMed.

Question: I am conducting a multistate analysis, and I am interested in using HCUP data. However, I require research-ready statistics as opposed to the full HCUP databases. I noticed a tool, HCUP Fast Stats, on the HCUP User Support (HCUP-US) Web site that appeared to be useful for my analysis. I am interested in learning more about this online tool.

More specifically—

What is HCUP Fast Stats, and how is it different from HCUPnet?
I am interested in timely data. What is the most current data year available through this tool?
Am I able to look at multiple years of data?
Is there an online methodology section that I can reference so that I know how the data was created?
Will I have the option to view the data by payer? What other characteristics are available?

What is HCUP Fast Stats, and how is it different from HCUPnet?

HCUP Fast Stats provides easy access to the latest HCUP-based statistics on health care information topics at State and national levels. Fast Stats uses visual statistical displays in stand-alone graphs, trend figures, or simple tables to convey complex information at a glance.

Fast Stats differs from HCUPnet, our free online query tool, because the statistics are more topic focused, whereas HCUPnet provides more general query pathways that use precalculated statistics from the HCUP databases. Fast Stats includes three specific topics:

State Trends in Hospital Use by Payer for Inpatient Stays and Emergency Department Visits
National Hospital Utilization and Costs by Inpatient Stays, Most Common Diagnoses for Inpatient Stays, and Most Common Operations During Inpatient Stays
Opioid-Related Hospital Use

Another difference between Fast Stats and HCUPnet is the timeliness of the data. Information in Fast Stats is updated regularly (quarterly or annually, as newer data become available). HCUPnet typically is updated as State and nationwide databases become available.

I am interested in timely data. What is the most current data year available through this tool?

The timeliness of the data in Fast Stats varies by topic and data type. The State Trends in Hospital Use by Payer and Opioid-Related Hospital Use topics are updated regularly, and both provide quarterly inpatient statistics, which are often more timely than annual statistics from the State Inpatient Databases (SID). The State Trends in Hospital Use by Payer topic, which was updated in November 2018, currently provides inpatient statistics for a subset of States through Quarter 2 of 2018.

The National Hospital Utilization and Costs topic typically is updated once a year following the release of the National Inpatient Sample (NIS).

Am I able to look at multiple years of data?

Yes. All topics in Fast Stats include multiple years of data, which are displayed in trend figures or tables. The initial starting year displayed will change as newer data become available, but users are to download a Microsoft® Excel export file for each topic, which includes all data from all years.

Is there an online methodology section that I can reference so that I know how the data was created?

Below the graphs or tables in each Fast Stats topic are five selections for users interested in obtaining additional information. The third selection, Show Data Notes & Methods, provides information on the definitions and methodology used as well as additional information about the data included in that topic. The same information also is included in the Excel export file.

Will I have the option to view the data by payer? What other characteristics are available?

Yes, all topics in Fast Stats provide an option to view statistics by the primary expected payer. Several other characteristics are also available for stratification. Within each topic, users can select from several characteristics contained in various drop-down menus, such as hospitalization type (e.g., maternal, mental health/substance use), patient characteristics (e.g., age, sex, location, community-level income), and setting of care.
