The 2005 Patient Safety and Quality Improvement Act (PSQIA) created the Network of Patient Safety Databases (NPSD), which captures, aggregates, analyzes and disseminates information on patient safety and quality submitted by providers and patient safety organizations (PSOs). This chartbook, which is the first publication of comprehensive patient safety data aggregated and analyzed under the PSQIA, characterizes the nature of hospital patient safety events and lessons learned.
