AHRQ’s Surveys on Patient Safety Culture (SOPS) are designed to help healthcare organizations assess how staff members perceive the ability of various aspects of organizational culture to promote a safe patient environment. The Surveys on Patient Safety Culture are a suite of surveys that assess provider and staff opinions about patient safety culture in hospitals, outpatient medical offices, nursing homes, community pharmacies, and ambulatory surgery centers. The patient safety culture surveys program at the Agency for Healthcare Research and Quality (AHRQ) supports the development and promotion of patient safety culture surveys, resources, and databases, and provides technical assistance to users.
