This report describes state-specific trends in primary cesarean delivery rates from 2006 through 2012 for reporting areas that implemented the 2003 U.S. Standard Certificate of Live Birth by January 1, 2006, and from 2009 through 2012 for reporting areas that implemented the 2003 revision by January 1, 2009. State-specific changes by gestational age are also explored.
