This commentary provides a clinician's account of near misses, medication errors, and care omissions that resulted in a family member's unnecessarily prolonged ventilation. The author argues for system approaches to address normalized complacency regarding complicationsfocused on changing culture, improving care continuity, and use of health information technology.
