Reporting adverse events in a war zone.

High-risk situations are prevalent in clinical environments. This commentary reviews the unique circumstances health care teams face in conflict or disaster zones that can contribute to adverse events. The author discusses how reporting what goes wrong in these settings can help aid agencies improve the safety of care delivery under challenging conditions.
