Applications Due by April 1 for 2019 Million Hearts® Hypertension Control Champions

Has your practice or health system achieved a blood pressure control rate of at least 80 percent for patients with hypertension? Applications are due by April 1 for Million Hearts® 2019 Hypertension Control Champions. Million Hearts®, an initiative co-led by the Centers for Disease Control and Prevention and the Centers for Medicare & Medicaid Services, aims to prevent 1 million heart attacks and strokes in five years. Since 2012, the initiative has recognized 101 Hypertension Control Champions serving more than 15 million adults.
