Medication appropriateness in vulnerable older adults: healthy skepticism of appropriate polypharmacy.

Patient age, disease state, and frailty may complicate medication regimens. This review examined the potential for polypharmacy to contribute to harm. The authors recommend a criteria set to be applied to guide clinician decision-making to determine appropriate polypharmacy in older patients to improve safety.
