NPSD Chartbook
The Network of Patient Safety Databases (NPSD) Chartbook provides an overview of the patient safety data captured in the NPSD through the AHRQ Common Formats for Event Reporting (CFER) and highlights patterns and trends in the data. The Chartbook represents the results of analyses designed to characterize the nature of patient safety events in hospital settings and represents a first attempt to collect this type of comprehensive data on a national scale. It examines data for topics that cut across the multiple modules in the AHRQ Common Formats for Event Reporting for Hospitals (CFER-H, versions 1.1 and 1.2
