The Centers for Medicare & Medicaid Services (CMS), Office of Minority Health presented the 2019 CMS Health Equity Awards to recognize organizations that demonstrate an exceptional commitment to health equity by reducing disparities among minority and other underserved populations.  The 2019 winners are HealthPartners, for their organization-wide activities to achieve health equity, and Centene Corporation, for their commitment to make sweeping accessibility improvements to their facilities.
This is the second year the agency has offered the annual CMS Health Equity Award, which identifies organizations working to achieve health equity for vulnerable populations and shines a light on their leadership to provide examples of successful approaches to reducing disparities.

Register at: https://www.resourcesforintegratedcare.com/GeriatricCompetentCare/2019_GCC_Webinar/Falls_Prevention_for_Older_Adults
Falling is not an inevitable part of aging.  Older adults with chronic conditions are at higher risk of falls,[1] making older adults dually eligible for Medicare and Medicaid particularly vulnerable [2],[3].  This webinar will provide an overview of the importance of falls assessment and falls prevention for older adults and their caregivers and offer concrete interventions and strategies for providers to improve mobility and prevent falls.
Intended Audience: This webinar is intended for a wide range of stakeholders - providers, health care professionals (such as home care agency administrators and staff), social workers, and front-line staff of Medicare Advantage plans, Medicare-Medicaid Plans (MMPs), Dual Eligible Special Needs Plans (D-SNPs), and PACE Organizations interested in learning more about falls prevention.
CME/CE Credit Information: CMS is accredited by the International Association for Continuing Education and Training (IACET) to offer continuing education (CE) credit.  The American Geriatrics Society is accredited the National Association of Social Workers (NASW) to provide continuing education for social workers and by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.
