The Commonwealth Fund created this tool so that healthcare organizations can assess the benefits and risks of investing in health-related social needs for patients with high-cost, complex conditions. This tool gives organizations the ability to determine whether the cost of providing nonmedical needs for patients will be lower in the long run than the healthcare costs of treating these patients.
