SummaryThe Hand-in-Hand Perinatal Palliative Care Program provides a formal, organized system of support for pregnant women and families who experience or will soon experience the loss of a baby with a life-limiting condition. A multidisciplinary team provides emotional support and compassionate, nonjudgmental care for the baby and family. With the assistance of an assigned team member, parents develop a birth plan and advance care plan that outlines desired medical interventions (if any), and all members of the health care team recognize and support these decisions. The team member helps parents create a memory box with tangible items that celebrate the baby’s life and honor his or her memory. He or she also involves other team members as necessary, such as the chaplain to provide spiritual support and the social worker to arrange hospice care and other services. Anecdotal feedback suggests the program is highly valued by the families it serves.
