Using Health IT to Measure Practice Transformation
The Agency for Healthcare Research and Quality (AHRQ) is hosting a webinar Wednesday, November 19th from 3:00-4:30pm ET, on models that have been used by Practice-Based Research Networks (PBRNs) to measure practice transformation. The expert panel will discuss models including: the TRANSLATE Model; the Solberg-Mold Practice Change/Quality Improvement Model; the Improving Primary Care through Health Information Technology; and the Vermont Integration Project.
