The accuracy of trigger tools to detect preventable adverse events in primary care: a systematic review.



    Trigger tools employ electronic health record data to automatically detect safety hazards. This systematic review assessed accuracy of the Institute for Healthcare Improvement Outpatient Adverse Event Trigger Tool. Although the tool identified preventable adverse events, it had a high false positive rate.
