CCSQ will host a Special Open Door Forum (ODF) to allow hospices and other interested parties to ask questions on the development of a Hospice Outcomes & Patient Evaluation (HOPE) Tool. This SODF is part of a series of regular SODF’s CMS plans to host on this patient assessment tool and other key topics related to the Hospice Quality Reporting Program.
