New Resource Features How to Implement Pharmacy-Based Medication Adherence Interventions for CVD Prevention

CDC’s Division of Heart Disease and Stroke Prevention has released a new resource to help communities and health systems implement CPSTF-recommended Tailored Pharmacy-Based Interventions to Improve Medication Adherence for Cardiovascular Disease Prevention.

The webpage highlights key intervention components and explains how tailored pharmacy-based interventions may be implemented as one component of a broader Pharmacists’ Patient Care Process. The page links to tools and resources from CDC and other partners that walk users through implementation steps, including identification, assessment, and action. There are also links to stories from the field and a brief slide presentation that can be downloaded and shared with colleagues and partners.
