    Incident and error reporting systems in intensive care: a systematic review of the literature.

      This systematic review found that incident reporting systems used in intensive care units failed to meet the standards recommended by the World Health Organization for adverse event reporting systems. The common problems with these systems—principally, insufficient analysis of reported incidents and inadequate feedback—have been noted in othercommentaries.
