Medication errors with antituberculosis therapy in an inpatient, academic setting: forgotten but not gone.



    This retrospective, single-site study found that 61% of patients receiving antituberculosis treatment had at least one medication error related to prescribing, administering, or monitoring tuberculosis drugs. The most common errors were due to inadequate dosing adjustments for weight, renal insufficiency, or drug–drug interactions.
