Understanding patient safety performance and educational needs using the 'Safety-II' approach for complex systems.

Approaches to preventing medical error have traditionally concentrated on learning from failure. This commentary discusses the value of drawing from excellence to improve outcomes in complex systems. The authors recommend focusing on both what went wrong and excellent performance to inform patient safety training and curriculum development.
