The Center for Devices and Radiological Health (CDRH) would like to announce that Form FDA 3640 – Reporting Guide for Laser Light Shows and Displays has been updated and can be accessed at the following web address:
The revisions to this report were made to clarify that the manufacturer of the laser light show described in this report is responsible for the information provided in the report and should provide their signature regardless of who completes and/or submits the report. The revisions also provide the laser light show manufacturer the option to address future communications to the person who submitted or prepared the report.
CDRH continues to encourage manufacturers to include an email address on any submission since it allows faster and more efficient communication.
Despite the expiration date listed on this form, this is the most current version available. CDRH will continue to use this form until an updated expiration date is issued.
