Adding a Medicare Authorized Representative
Medicare can't give your personal health information to anyone unless we have permission in writing first. There are 2 ways to give permission: (1) Print and fill out form CMS-10106: "Authorization to Disclose Personal Health Information" and then mail it to us.
(2) Login to your Medicare account to fill out and submit online.
Don't have a Medicare account? Creating an account is easy and gives you electronic access to your health information. Once you're logged into your account, follow these steps to fill out and submit the form:

- Select your name in the top right corner of the page.
- Select "My account."
- Select "Manage my representatives."
- Select “Medicare Authorization to Disclose Personal Health Information form."
- Enter the requested information and select the "Continue" button.

About Continuing Education Units (CEUs)
The number of CEUs you can earn is 0.2 (2 hours) for each day you attend the training and complete all course requirements. You can access the link to your certificate from the same page you used to complete the requirements. A description of the requirements to earn CEUs for these training events is found on the registration page for each webinar.

The Medicare General Enrollment Period and Medicare Advantage Open Enrollment Period are happening now! Both enrollment periods are from January 1 – March 31. For more information, go to Medicare.gov/sign-up-change- plans/joining-a-health-or- drug-plan.
January is National Glaucoma Awareness Month. Medicare covers a glaucoma test once every 12 months for people at high risk for glaucoma. All people with Part B who are at high risk for glaucoma are covered. Visit Medicare.gov/coverage/ glaucoma-tests.html for more information.
