Improving Communication at Discharge
AHRQ’s Patient Safety Network (PSNet), which highlights journal publications, books and tools related to patient safety, recently featured the article: Improving communication with primary care physicians at the time of hospital discharge. The article describes an intervention that led to an increase in verbal communication with primary care physicians at discharge.
