MHS Patient Safety Data Snapshot
The MHS Patient Safety Data Snapshot is a monthly compilation of two types of patient safety data:1) Sentinel Event notifications submitted to the Patient Safety Analysis Center
2) Anonymous, voluntarily reported patient safety events via the web-based incident reporting system known as JPSR
This publication provides an MHS enterprise-level snapshot of trends in SEs and JPSR events so that providers and health care administrators can help identify areas for patient safety improvement in their facility and throughout the MHS.

The Institute for Healthcare Improvement/National Patient Safety Foundation (IHI/NPSF) called upon the IHI/NPSF Patient Safety Coalition, a diverse group of stakeholders from across the continuum of care, to develop the toolkit, “Optimizing a Business Case for Safe Health Care: An Integrated Approach to Safety and Finance,” for helping patient safety leaders optimize a business case for safe care. This toolkit specifically emphasizes the need for collaboration among safety and financial leaders to demonstrate the value and return on investment for safer, quality care.
The project co-chairs, Mr. Doug Beighle and Dr. Jane Englebright, will provide an overview of the toolkit components, including a close look at the recommendations and resources for preparing and presenting successful business cases. The toolkit will be available June 26, 2017, on www.ihi.org.

On-Demand eLearning: Root Cause Analysis*
This self-paced module outlines the DoD Patient Safety Program’s suggested practices for conducting a root cause analysis, from the initial reporting of a patient safety event through the formation of the RCA Team to the identification of contributing factors and root causes and the recommendation of corrective actions.
*Denotes that activity is eligible for Continuing Education credit. All CE credits are managed by the Postgraduate Institute for Medicine.
