How do physicians conduct medication reviews?

This study analyzed audiotaped discussions of primary care clinic visits to analyze how physicians discuss medications with their patients. Physicians were able to comprehensively review each medication with patients only rarely, with most discussions focusing on the efficacy of the medications. Comprehensive medication review by pharmacists has been shown to decrease medication errors at the time of hospital discharge.
