Want to help people with Medicare stay healthy and prevent diabetes? Become a Medicare Diabetes Prevention Program (MDPP) supplier. As a supplier, you’ll help people with Medicare make lasting behavior changes through group-based training & individual support. Read below for more information on how to become an MDPP Supplier. STEPS TO BECOME A MEDICARE DIABETES PREVENTION PROGRAM SUPPLIER:

Achieve CDC Recognition


Apply to the Centers for Disease Control and Prevention (CDC) Diabetes Prevention Recognition Program (DPRP). Submit an application online to become a CDC-recognized organization. The process is handled by CDC.
Offer the program for 12 months. Offer group-based sessions to help participants eat healthier, lose weight, and increase physical activity. Track and report participants’ attendance, weight loss, and exercise minutes.
Submit evaluation data to CDC every 6 months. Continue to track and report participants’ progress. Your organization can achieve Preliminary or Full recognition after 12 months by meeting certain standards, such as participant attendance and weight loss.

Apply to Become a Supplier


Once you get Preliminary or Full CDC recognition, then you can apply to become an MDPP Supplier. Submit an application online or complete a print application to enroll a supplier. The approval process usually takes at least 80 days and is handled by Medicare.
Provide Medicare Diabetes Prevention Program Services. Offer group-based sessions and coaching using a CDC-approved curriculum to help beneficiaries learn how to get more exercise, choose healthy foods, and lose weight.
Bill Medicare. Submit claims to Medicare and get paid up to $670 when participants meet attendance and weight loss goals over the course of 2 years of sessions.
